If you would like to have more information about the LSW service, please fill out the simple questionaire in the next section. It does not ask for any confidential information. However, it will allow me to determine if I have relevant experience that would allow me to be of service to you.

After the completed questionaire is received I will respond to you, as you direct, to let you know if I think I can be of service to your organization. It is my philosophy to choose my clients carefully to make sure that my time is used in the most productive manner. I expect you to be careful in selecting me as an advisor because I will be careful in selecting you as my client.

My response will provide you with:

• An outline of my experience that is relevant to you and your organization.
• A copy of the Confidentiality and Non-Compete agreement that will be signed by Len and given to you prior to a first in-person interview.
• A sample proposal.

After you receive my response it will be up to you to contact me if you would like to discuss LSW services in more detail. I will not contact you. If you decide to request more information, a telephone conversation is a good start, however at least one in-person interview is required before I will submit a formal proposal.
